How it works
Three steps, at your pace
Housing stabilization moves through three stages. You can stop and ask questions at any point along the way.
Consultation
A housing consultant sits down or speaks with you to see whether you meet the eligibility requirements for HSS, and helps you obtain the proper documents if you need them.
Transition
We help you find housing of your choice, then assist with the move itself.
Sustaining
We follow up to see how things are going in your new home, and keep helping you work through the challenges that could otherwise cost you your housing.
Before you begin
Who is eligible for Housing Stabilization Services?
- A person with a disability
- Must have Medical Assistance
- Must be 18 or older
- Experiencing housing instability, or
- In a living situation that is provider-led, and you want to move into a more independent setting
- Adults who have a waiver such as CADI, DD, BI, EW or CAC
